Claims Form

For your convenience, we have provided an on-line version of our Claim Form to expedite your claim process. Please complete the claim form that follows.

As per the Contract Terms and Conditions (located on your Bill of Lading), Long Distance and Interstate Move claims must be submitted within 9 months of the delivery date.  Local Move claims must be submitted within 90 days from the delivery date.

Receipt of your claim will be acknowledged and a Claim Adjustor will be assigned. Your Claim Adjuster will review the claim and contact you, if necessary, with any further instructions.

Since all damaged items are subject to inspection, please do not proceed with any repairs and do not dispose of any damaged items. DO NOT remove from the premises any item for repair estimates as the carrier would not be the last physical handler of the items and this would void the claim for those items.  A “comments” section has been provided at the end of the claim form should you need to provide further explanation for any items or issues referenced in your claim.

Before presenting a claim for loss or damage, review the terms and conditions in the last paragraph on the back of the Uniform Household Goods Carrier’s Bill of Lading and Freight Bill. Please do not proceed with any repairs, obtain replacements, relocate items, or discard any items as these actions nullify your claim. You will be contacted by a Reebie adjuster regarding the action to be taken toward the settlement of your claim. Your adjuster may enlist the services of an independent, professional furniture restoration service in your area to view the damages claimed. Reebie does reserve the right to a visual inspection of all damages claimed. The services that assist us are not owned or operated by Reebie or Allied Van Lines, but are selected based upon their craftsmanship and reputation in your area. Upon the completion of these processes, your Reebie adjuster will contact you regarding the details of your claim. In all cases, keep damaged articles including shipping containers for inspection. Arrangements may need to be made to inspect and estimate damage to the articles you have claimed.
